We require all new and returning patients to provide insurance and medical information prior to each appointment. You can expedite the check-in process on the day of your visit by downloading, printing, and completing the required forms in advance of your appointment.
We accept MOST insurance, however to avoid billing issues, it is HIGHLY recommended that you check with your insurance carrier for provider participation as some carved out plans we do not participate with.
We DO NOT accept:
Straight Medicaid, UHC Plan, Tricare, Empire BCBS, GHI, Independent Health, Fedelis, Wellcare, VA Choice, Health Republic, and Western BCBS HMO plan Worker's Comp and MVA/No Fault.
